Definitely had that issue with an intake hose before. Acted pretty much as you describe. If it's of unknown age and/or soft, I would replace it.

The manifold studs should just pass through the turbine housing holes and be secured with a nut. Not sure why you'd want to twist anything on.

Any of those options should do ok. As mentioned above, a boost gauge and wideband are necessary tools when raising the boost. Fwiw, I did need injectors at around 15 psi on my 16t.
