Upper hose is hot and the lower is cold. Time to replace the radiator?

Upper hose is hot and the lower is cold. Time to replace the radiator?
How long had it been running? if you look under cap (when cold) is there lots of crud?Upper hose is hot and the lower is cold. Time to replace the radiator?
Thinking I should just do a radiator and water pump anyway.
The thermostat may may have been installed wrong, will that cause it to overheat?
